I have received a police letter through my door and need to translate it

Hi all, I got a slip through my door this morning from the police. It's obviously in Spanish and I don't understand it. Would anyone be so kind to tell me what it is? I will pm it to anyone that wants it :)

Try putting it into Bing.com to translate it.
Not 100% accurate, but it will give you a general idea of what it is saying.
