My understanding is that the NHS is a resident based service. So you have to be ordinarily resident in the UK to get free treatment from the NHS.

If you receive healthcare in Spain through your S1 form, You will need to apply for a UK issued EHIC. The phone number of the Overseas Healthcare Team is 0044 (0)191 218 1999.
This is not the same EHIC card that is issued to UK residents as the charge back goes to the country registered with the S1.
